Revisional Ptosis (Drooping Eyelid) Surgery - Dr. Kami Parsa, Beverly Hills

A 24 year old female underwent a cosmetic procedure to reduce her frown lines and also eliminate a scar on her forehead that she got as a kid.

Using a newly developed technique, the plastic surgeon went in through her eyelids to get to the scar tissue and release the brow muscles.

While recovering she noticed that she had lost the use of her right upper eyelid. This caused her a loss of visual field because her drooping eyelid was covering nearly 50 percent of her eye.

Alerting her surgeon to this problem, a 2nd surgery was scheduled to identify and correct the cause of the ptosis. Following her 2nd surgery, the ptosis only became worse.

Realizing there was a serious breakdown in care, she began researching other doctors who specialized in people born with this condition and quickly scheduled a consultation with oculoplastic and reconstructive surgeon Dr. Kami Parsa.

During her initial consultation with Dr. Parsa, he immediately reconized what the anatomical problem was and ured her that is was something that could be fixed. Following her surgery with Dr. Parsa her expectations were blown out of the water and she now has 100% recovery of her eyelid.

Ptosis, pronounced toe-sis, is the medical term for drooping of the upper eyelids. The droopy eyelid can be mild to severe. People who have ptosis may complain that people tell them they look “tired or lazy.” Because of a constant effort to raise the eyelids it is not uncommon to also complain of fatigue and tension headaches.

Surgical correction of aging face requires a thorough evaluation of structures and designing of specific procedures for specific individuals.. Basically facial rejuvenation falls into two major groups. One is when no tissue is stretched physically by surgical means and effects of smoothness and tightening is achieved by laser, Botox, thermage, chemical peel and fillers. In other group redundant skin, SMAS and ligaments are tightened and or reduced. The second group falls in two further categories:-
1-Suspension of tissue without synthetic material.
2-Suspension with synthetic material
Suspension without synthetic material provides maximum facial expression and mobility. This is attributed to natural elasticity of tissue this effect can be achieved in all areas of face, neck and temporal region. Suspension with synthetic material regarding whole face and neck with or with out tissue reduction gives static look and has limited application. One of the most important objectives of face-lifting is to preserve facial expression. Any static material used for upholding the kinetic facial constituent reduces its mobility. More we use static materials more we reduce facial expression. Till yet we have not come across to a suitable synthetic material for suspension. Prolene is plastic in nature. It has no elasticity as we have in normal tissue. Its pulling effect is lost very quickly. Nylon has elasticity but can not be compared with normal tissue. We have to wait for suspension with synthetic material until we find a suitable material comparable to normal tissue for better facial expression.
The redundant skin should be pulled where gravity has acted most in lifetime.. Gravity acts both vertically to face (in standing position) and horizontally to face (in sleeping position). Though in sleeping positions it varies and has not the same effect as in standing. Each case should be scrutinized on individual basis at the time of operation
